Clinical Review of 874 Patients with Gastric Cancer




Abstract
In the western countries, the incidence and mortality of the gastric have been decreased, but the gastric cancer is the most frequent malignancy in our country. Despite recent advances in diagnosis and treatment, gastric carcinoma remains a major
cause
of death in the world. The clinicopathologic profile of 874 consecutive patients who underwent primary gastrectomy during 1981~1992 at Chonnam University Hospital were reviewed and analyzed.
The most prevalant stage of gastric carcinoma was stage IV, and the incidence of stage I has increased steadily. The rate of early carcinoma in the recent period of 1987~1992 was 16.3% and that in the past period of 1981~1986 was 10.1%. The most
common
gross type of early gastric cancer was type IIc.
The antrum was most frequently involved site of gastric cancer up to 64%. The cardiac involvement of gastric cancer was increased recently. The incidence and the location of lymph node metastasis were increased and further from the primary cancer
according to the invasion and the stage of majority of synchronous gastric cancers(65%) was stage I.
The resectabe rate increased from 90.6% to 94.7%, and curative resectabe rate also increased from 63.4% to 75.9%. By the operation methods, recently the total gastectomy was increased from 9.2% to 19.2%. combined resection of other organs was
performed
about 14.2% and most common resected organ was spleen. The liver and peritoneal metastasis was increased according to the invasion depth. Recently we performed resection of liver and regional chemotherapy through hepatic artery.
The overall incidence of DNA aneuploid was 46.9% among the 175 patients Aneuploid incidence occording to TNM stage was 31.7% in stage I, 43.8% in stage II, 50.8% in stage III, ad 60% in stage IV. Aneuploid incidence was increased according to the
stage.
